Your registration has been received. Please do not register more than once, you will receive a copy of your registration in the email that you supplied.  We will contact you upon your eligibiltiy and in the order that we receive the registration.

You will receive a separate email with your appointment time and date as well as an attached consent form. Please fill out and print the first two pages of the attached consent form once received.

Please give us 48 hours to respond. Note that you will receive an email from COVID@ridgewoodnj.net. Appointments will be scheduled on November 10th or November 18th beginning at 3:30pm depending on vaccine availability.
